Why do I get blood relation questions wrong at the fourth link?

Because the sentence structure usually inverts there. 'A's father's sister' and 'the sister of A's father' read the same to a tired eye but are parsed from opposite ends. Draw the tree instead of holding it in your head: one symbol per person, an arrow for each stated link, and never a shortcut.

How should I handle a pointing-at-a-photo question?

Read it backwards. 'He is the son of my grandfather's only son' resolves from the innermost phrase outward: grandfather's only son is the speaker's father, so the man is the speaker's brother. Working outward-in is what produces the classic off-by-one-generation answer.

Is gender always determinable in these questions?

No, and assuming it is a common trap. 'A's child' does not fix a gender, so any option that needs one is unsupported. If a question can be answered without knowing a gender, the paper usually intends exactly that, and the option demanding it is the distractor.
